SUMMARY: Facilitates pharmaceutical care services, direct patient care programs, medication utilization systems within assigned services and care areas to assure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Works as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide patient-centered care
- Identifies high-risk patients and implements interventions to improve quality and safety
- Makes appropriate evidence-based, patient-centered medication recommendations
- Provides patient centered medication review, reconciliation, and counseling services as appropriate
- Actively identifies practice related issues which require evaluation and facilitate clinical research projects, quality improvement initiatives, or healthcare provider education as needed to advance practice
- Completes critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, sub therapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures
- Communicates effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc.) and assures continuity of pharmaceutical care between shifts and among staff
- Conduct clinical research and practice advancement projects, as well as patient care quality and regulatory compliance initiatives designed to improve medication use processes within the LTBB Health Department
- Provide medication and practice-related education and training to clinicians and other department staff
- Participate in continual quality improvement by monitoring processes, analyzing data, implementing interventions, and evaluating the effectiveness of those interventions.
- Assist in the development of a comprehensive prescription drug monitoring system within the LTBB Health Department using data from patient medical records, pharmacy data, and state PDMP data.
- Assist clinical staff in implementing and monitoring controlled substance agreements and patient medication adherence
- Assist clinical staff in continued medication management of chronic pain and work to identify at risk patients that may need additional support or alternative treatment options
- Assist LTBB in creating a workgroup of stakeholders to examine prescribing practices and trends to limit instances of overprescribing and reducing
- Assist in creating an INR management procedure to manage clinical patients on Coumadin
- Develop comprehensive policies, procedures, and infrastructure changes to create system wide improvements in how LTBB’s health agencies work together to address prescription drug misuse/abuse.
- Documents all clinical activities in patient medical records.
- Assists in the maintenance of accurate medication lists and allergies in patient medical records.
- Collaborates with other health care professionals to plan, monitor, review, or evaluate the quality or effectiveness of drugs or drug regimens; providing advice on drug applications or characteristics.
- Provides pharmacy services as last resort if short staffing situations occur in LTBB Pharmacy.
- Maintains professional knowledge base through completion of continuing education activities.
- Conducts behavior in a manner that is professional and courteous to all patients and staff at all times.
- Consistently portray a positive attitude that fosters a pleasant work environment.
- All other position related duties as assigned by supervisor.
KNOWLEDGE, SKILLS, AND ABILITIES: Must possess compassion and high level of service for our patients, parents, and staff. Have integrity and always doing the right thing. Must have team building, organizational, staff development, interpersonal and communication skills. Ability to organize, work and engage in a variety of tasks simultaneously. Must have a high degree of initiative, accountability, independent judgment, and a commitment to confidentiality. Knowledge of potential side effects of certain drugs or medications and able to properly advice and counsel patients. Ability to communicate and motivate patients regarding disease state, desired outcomes, proper administration, use, actions, precautions, and storage of their medications. Strongly preferred training and experience in culturally diverse environment preferably with Native American population.
EDUCATION AND EXPERIENCE: Pharmacy degree from an accredited institution. Current and unrestricted Pharmacist and Controlled Substance licenses from the state of Michigan. 2 years’ experience in a clinical setting required. Experience in a primary care setting preferred.
PHYSICAL DEMANDS: Standing for long periods of time, frequent walking, and occasional lifting of 10 to 20 pounds, reaching, bending, kneeling, or stretching is to be expected.
WORK ENVIRONMENT: Work is performed in a climate controlled medical office environment. The employee’s work schedule may need to be flexible and adjusted according to business need. The employee must be able to effectively handle stressful and compromising situations while remaining focused and professional, as well as able to deal with difficult people without losing perspective. Work may involve exposure to infections, communicable diseases, contaminated materials, bodily fluids, and radiation resulting in the need to wear personal protective equipment (PPE) such as masks, eye protection, rubber gloves and gowns. Occasional overnight travel may be necessary to attend meetings or trainings.
COMMENTS: Indian preference will apply. Candidates must pass a comprehensive background investigation. Individual must take a TB test, and get annual influenza vaccination due to working in a healthcare environment.
ABOUT US: On Sept. 21, 1994, the Little Traverse Bay Bands of Odawa Indians (LTBB) was federally reaffirmed with the signing of Public Law 103-324. The Tribe is governed by a nine-member Tribal Council who serve staggered terms. The LTBB Government presently employs over 200 employees across more than 20 departments, ranging from Healthcare to Natural Resources.
